About Course

Primary Health Care (PHC) and Community-Based Health Care (CBHC) form the foundation of effective health systems and are the main strategy for achieving Universal Health Coverage (UHC) and the health-related Sustainable Development Goals (SDGs).

This course equips health professionals—especially nurses, midwives and community health workers—with the knowledge, practical skills and professional attitudes required to plan, implement and evaluate community-oriented health services in Uganda and similar settings.

PHC emphasizes equity, prevention, health promotion and community participation, while CBHC provides the practical framework for involving people in identifying their own health needs and mobilising local resources.

Through this course you will explore the principles, policies, and practical methods for delivering essential health care at the community level and for engaging families and local leaders in sustainable health action.

Course Aim:

To develop competent health professionals who can effectively implement Primary Health Care and lead Community-Based Health Care initiatives to improve the health and well-being of individuals, families and communities.

Learning Objectives:

By the end of the course, students should be able to:

  1. Explain key concepts and principles of PHC, including equity, participation and appropriate technology.

  2. Outline the history of PHC, from Alma-Ata to the current Universal Health Coverage agenda.

  3. Identify major determinants of health and their impact on community well-being.

  4. Apply basic epidemiology to disease prevention and control in the community.

  5. Carry out community health assessments and situation analysis using participatory methods.

  6. Plan and implement community-based interventions through organization, mobilization and empowerment.

  7. Demonstrate skills in home visiting, school health programs and other CBHC services.

  8. Work with other sectors to promote population health and sustainable development.

  9. Evaluate community health programs using key indicators of participation and effectiveness.

  10. Show professional attitudes and ethics such as teamwork, responsibility and respect for community values.

Expected Competencies

On completion, graduates will be able to design, deliver and evaluate PHC and CBHC programs, promote health at individual and community levels, and contribute to the achievement of Universal Health Coverage and the SDGs in Uganda and beyond.

Topic 1: Foundational Principles of Primary Health Care (PHC)
Primary Health Care (PHC) forms the foundation of every effective health system and is the main entry point for individuals, families and communities to access health services. This topic explores the philosophy, key concepts and guiding principles of PHC, tracing its historical development from the 1978 Alma-Ata Declaration to today’s goal of Universal Health Coverage (UHC).

This topic explores Community-Based Health Care (CBHC), an approach that brings health services directly to communities, emphasizing prevention, health promotion, and active participation of community members. It highlights how communities can identify health problems, plan and implement solutions, and sustain interventions, all while being supported by the formal health system.

Topic 4: Specialized CBHC Services and Practice
This topic explores specialized Community-Based Health Care (CBHC) services and practice, focusing on critical skills and interventions for vulnerable groups and specific community settings. It emphasizes practical approaches, such as home visiting, to assess household health needs, promote healthy behaviors, and support families in improving well-being.
